How Health Information Supports Addressing the Social Determinants of Health

This moderated discussion will include the differences in social determinants and social needs. Presenters will also explore how if EHRs were to start collecting information on social needs and determinants, patient health data could inform social determinants of health policy decisions at a community and population level.

The public health perspective will inform health information professionals about what is still needed in health and public health data, how that data can and should be used, and how personal health and public health intersect. The discussion will also touch on how social determinants of health have been affected by COVID-19.
